PROXY FORM for ANNUAL GENERAL MEETING

However, our Constitution provides a limited right to vote by proxy for two purposes only:

1. Approving the audited financial statements of the Congregation.

2. The appointment of auditors.

If you are unable to personally attend the general meeting scheduled for May 29, 2025 but wish to help ensure that the required quorum of 50 Members will be available to deal with the afore-mentioned items, please complete and return the form of proxy to the Synagogue office by May 23 by mail, or by email to michael@adathisrael.com. Note that each adult member is entitled to one vote.

The undersigned Member of Adath Israel Congregation (the “Congregation”) hereby appoints Barry Green, Board Chair of the Congregation, or INSTEAD OF EITHER OF THE FOREGOING,

as the nominee of the undersigned to attend and act for and on behalf of the undersigned at the general meeting of the Members of the Congregation to be held on the 29th day of May, 2025, and at any adjournment or adjournments thereof, FOR THE PURPOSE OF APPROVING THE AUDITED FINANCIAL STATEMENTS OF THE CONGREGATION AND FOR THE PURPOSE OF APPOINTING THE AUDITORS OF THE CONGREGATION, AS WELL AS RATIFICATIONS OF ACTS OF THE BOARD, AND FOR THESE PURPOSES ONLY, to the same extent and with the same power as if the undersigned was personally present at the said meeting or such adjournment or adjournments thereof.
