PROXY FORM for ANNUAL GENERAL MEETING
(MAY 30, 2024)
 

To vote on any question at a meeting of our members, their membership status must be in good standing. For most matters, the Member must be personally present at the meeting in question.

However, our Constitution provides a limited right to vote by proxy for two purposes only:

1. Approving the audited financial statements of the Congregation.

2. The appointment of auditors.

If you are unable to personally attend the general meeting scheduled for May 30, 2024 but wish to help ensure that the required quorum of 50 Members will be available to deal with the afore-mentioned items, please complete and return the form of proxy to the Synagogue office by May 24th by mail, or by email to ronly@adathisrael.com. Note that each adult member is entitled to one vote.

The undersigned Member of Adath Israel Congregation (the “Congregation”) hereby appoints Robbie Feldman, Incoming President of the Congregation, or failing him, Barry Green, Incoming Board Chair of the Congregation, or INSTEAD OF EITHER OF THE FOREGOING,

as the nominee of the undersigned to attend and act for and on behalf of the undersigned at the general meeting of the Members of the Congregation to be held on the 30th day of May, 2024, and at any adjournment or adjournments thereof, FOR THE PURPOSE OF APPROVING THE AUDITED FINANCIAL STATEMENTS OF THE CONGREGATION AND FOR THE PURPOSE OF APPOINTING THE AUDITORS OF THE CONGREGATION AND FOR THESE PURPOSES ONLY, to the same extent and with the same power as if the undersigned was personally present at the said meeting or such adjournment or adjournments thereof. THIS PROXY IS SOLICITED ON BEHALF OF THE BOARD OF GOVERNORS OF THE CONGREGATION. MEMBERS HAVE THE RIGHT TO APPOINT ANOTHER MEMBER TO REPRESENT THEM AT THE MEETING OTHER THAN THE NOMINEES DESIGNATED ABOVE AND MAY EXERCISE SUCH RIGHT BY INSERTING THE NAME OF THEIR NOMINEE IN THE BLANK SPACE PROVIDED ABOVE FOR THAT PURPOSE.
